Women's 4 On 4 Ball Hockey League – Player Participant Form

This form must be completed in its entire by the participant or legal guardian [for participants under the age of 18] registering with NLBHA to participate in sanctioned programs, leagues, exhibition games, tournaments, provincial championships, national and international events.

Any player under the age of 16 must get prior approval from Ball Hockey NL Female Coordinator Mike Hipditch via this link https://ballhockeynl.ca/womens/

Location: Northpoint Sports Facility located on 42 O’Leary Avenue, St. John’s

Day/Time: Saturdays between 1:00 PM to 3:00 PM - [Depending on the number of participants]

Season Length: 11 Games guaranteed. [including playoffs]

Start Date | End Date: Saturday January 06, 2024 to Saturday March 16, 2024

Game Length: 2 X 23 min periods. [run clock]

Team Selection: Drafted by pre-selected Team Captains with assistance from organizers.

Season's End Banquet @ The Bigs Ultimate Sports Grill [Freshwater Rd Location].

Banquet Date: 4:00 PM on Saturday March 16, 2024. [if no game cancelations]

Cost: $160.00 per player plus $20.00 [Refundable Jersey Fee]

Insurance Fee: $15.00 [19+] per player and $10.00 [18 Under] per player, players who were registered for a sanctioned NLBHA/CBHA event/league or program after April 1, 2023 will not have to pay for the insurance fee for the *Winter 2024 season. Please select the option that applies to you.

Note: If you select the incorrect option, the participant in question will be assessed a $10.00 admin fee + the insurance fee that applies for this infraction. If you are not 100% sure that you are already covered under the NLBHA Insurance policy, please contact us via email at info@hockeybuds.ca

The onus is on the participant to make sure that they qualify for the no charge insurance fee prior to submitting their participation form.

Important: Once a participant (athlete, official, volunteer) or legal guardian completes registration form and submits, the registration form needs to be verified before the process is complete. If not complete or incorrect, an email will be sent to participant (or legal guardian) advising them of the issue that needs to be resolved. A $10.00 Admin fee will be charged for any errors that need to be resolved thus it is very important that the participant/legal guardian reviews everything carefully and completes the required fields correctly.
